Postpartum Pelvic Floor Recovery: What’s Normal vs. When to Seek Help

A woman with outstretched arms celebrates the sunset in a grassy field, capturing the essence of freedom and joy.

Emerge Physical Therapy and Wellness – Lincoln Park, Chicago

Bringing a baby into the world is a life-changing experience — physically, emotionally, and mentally. But after delivery, many new moms are left wondering:
💭 “Is what I’m feeling normal?”
💭 “Do I just need more time, or should I see a specialist?”

The truth is, every woman’s postpartum recovery is different, but there are clear signs that your pelvic floor may need extra support. At Emerge Physical Therapy and Wellness in Lincoln Park, we help new moms confidently return to the activities they love — leak-free, pain-free, and strong.

0–6 Weeks: The Early Healing Phase

What’s normal:

  • Mild soreness or pressure in the pelvic area

  • Light spotting or discharge

  • Feeling “weak” in your core and pelvic floor

  • Fatigue and difficulty holding in gas/urine (short-term)

Gentle things you can do:

  • Deep breathing (diaphragmatic) to reconnect with your core and pelvic floor

  • Short, slow walks to promote blood flow

  • Rest — your body is doing a lot behind the scenes

🚫 Avoid: Heavy lifting, crunches, jumping, or high-impact exercise.

6–12 Weeks: Regaining Strength

By this stage, most women have their first postpartum check-up. Unfortunately, pelvic floor function isn’t always assessed — but it should be.

You may still notice:

  • Leaking urine when sneezing, laughing, or exercising (stress incontinence)

  • A heavy or “falling out” sensation in the vagina (possible prolapse)

  • Ongoing pain with intercourse

  • Abdominal separation (diastasis recti)

When to see a pelvic floor PT:
If these symptoms continue past 6–8 weeks, they’re not “just normal mom life” — they’re signs your pelvic floor needs guided rehabilitation.

The Role of Pelvic Floor Physical Therapy

At Emerge Physical Therapy, we use a combination of:

  • Individualized exercises to strengthen and coordinate your pelvic floor and core

  • Manual therapy to release scar tissue or tension from birth

  • Education on posture, lifting mechanics, and breath to reduce strain on healing tissues

  • Return-to-activity plans so you can safely run, lift, and play without fear of leaking or injury
